Family Tree, Inc.
Enterprise Zone Contribution Tax Credit
Colorado taxpayers who make donations to Family Tree may be eligible to receive an Enterprise Zone contribution tax credit. Procedures for making enterprise zone contributions are detailed below:
1. Making a monetary contribution
All monetary contributions exceeding $200, and made to Family Tree during a calendar year, may be eligible for a 25% Colorado income tax credit. These contributions should be in the form of a check, when possible, and made payable to Family Tree, Inc.
Payments for auction items, or for the purchase of raffle tickets, are not eligible for an Enterprise Zone contribution tax credit through Family Tree.
These contributions will be used to support the economic development plan through direct and indirect job creation and job preservation; training, improvement of quality of life among low and moderate-income residents; and support the long-term viability of the community.
2. Completing the Colorado Department of Revenue form
The donor should complete the top portion (taxpayer name, address, and telephone number) of the Colorado Department of Revenue form entitled “Certification of Qualified Enterprise Zone Contribution.”
The taxpayer name must match that of the person or entity making the monetary contribution.
3. Mailing the contribution and certification form
The contribution check and certification form should be mailed to: Family Tree, Inc., 3805 Marshall Street, Wheat Ridge, CO 80033.
4. Taxpayer receipt of approved certification form
Family Tree, Inc. will complete the appropriate portions of the certification form and a copy of the approved certification will be mailed to the donor.
